01A note on using any of this

Every template below is a starting point, not a finished document. A permit form that has not been adapted to your equipment, your voltage classes, and your approval chain is a form someone will fill out without thinking — which is the opposite of what a permit is for.

Adapt them. Put your company name on them, change what needs changing, delete what does not apply, and have someone who knows your facility review them before they go into service. That is not a disclaimer; it is the actual instruction.
