Who enforces electrical safety in Indiana, what IOSHA actually cites here, what happened in real Indiana electrocution and arc flash cases, why the state's licensing patchwork matters to your program — and how the 2027 edition lands in steel, automotive, RV, pharmaceutical, logistics, and data center work.
Indiana classes are the ones where the licensing question always comes up first — because there is no state license, and half the room has worked under three different city rulebooks. It's also the state where the industrial history is standing right outside the training-room window: the mills of the Calumet region, the RV lines in Elkhart, and now data center steel going up in cornfields. The common thread I press in every Indiana class: none of those rulebooks — city license, building code, IOSHA — writes your safe work procedures for you. That's the employer's job, and 70E is the manual for it.
01Who enforces electrical safety in Indiana
Indiana is a full state-plan state. Workplace safety is enforced by the Indiana Occupational Safety and Health Administration (IOSHA), a division of the Indiana Department of Labor headquartered in Indianapolis — not by federal OSHA. The plan received initial federal approval in 1974 and final approval in 1986, and it covers both private-sector employers and state and local government employers. Only federal employees and a few federally regulated niches remain under federal jurisdiction.
For electrical safety, the practical picture looks familiar: Indiana adopts the federal standards by reference — 29 CFR 1910 Subpart S (1910.331–.335), 1910.137 (electrical protective equipment), 1910.147 (lockout/tagout), 1910.269 (electric power generation, transmission, and distribution), and 1926 Subpart K for construction — with only limited Indiana-specific departures (the state maintains its own excavation standard, for example). NFPA 70E remains what it is everywhere: the recognized method of compliance for electrical safe work practices, and the yardstick IOSHA and litigants hold your program against after an incident.
Three consequences of the IOSHA arrangement that Indiana employers routinely get wrong:
- "70E isn't law" doesn't protect you. IOSHA cites the same electrical work practice standards federal OSHA does, and the Indiana Occupational Safety and Health Act carries its own general duty obligation. When an incident occurs and your program doesn't hold up against 70E, that gap becomes the citation narrative — the arc flash case in Section 3 is a straight-line example, cited under 1910.333 and 1910.335.
- Public-sector workers ARE covered — the opposite of neighboring federal-plan states. Because Indiana runs a full state plan, employees of Indiana cities, counties, school districts, municipal utilities, and state agencies fall under IOSHA enforcement. - Low historical penalties are not low risk. IOSHA's fatality penalties have drawn public criticism for being small — several of the cases below settled in the four-to-five-figure range, and state legislators have proposed automatic minimum fines for fatal willful violations. Do not let a $7,000 headline number calibrate your risk. The real exposure is the incident itself, the repeat-violation multipliers that follow you across sites, and the civil litigation that uses the citation record — with your program measured against NFPA 70E — as its roadmap.
02Indiana by the numbers
Indiana's fatality count is mid-pack nationally, but its mix is not: Indiana is one of the most manufacturing-intensive states in the country, and manufacturing, construction, utilities, and agriculture all put workers near energized equipment daily. Nationally, the utility industry (0.75 deaths per 100,000 workers) and construction (0.73) have electrical fatality rates roughly seven times the all-occupation rate of 0.11 — and electricians as an occupation sit at 2.89 per 100,000, twenty-six times the average worker's risk (ESFI analysis of BLS data, 2011–2024).
Two more national findings that should shape an Indiana program:
- 70% of workplace electrical fatalities happen to non-electrical workers — laborers, roofers, maintenance welders, farmhands, equipment operators. Look at the Indiana cases below: a maintenance welder on a crane, a roofing crew, a boom truck operator. If your 70E training only reaches your electricians, it's missing most of the people who die from electricity.
- Hispanic and Latino workers carry a disproportionate share of electrical fatalities (0.18 per 100,000 vs. 0.10 for white non-Hispanic workers, per ESFI). Indiana's construction, RV, and food processing workforces include large Spanish-speaking populations — if your training isn't delivered effectively in Spanish, it isn't delivered.
The Indiana Department of Labor publishes CFOI fatality counts by major event category; the "exposure to harmful substances or environments" category (25 Indiana deaths in 2024) includes electricity along with heat, drugs, and chemical exposures. 03Real Indiana cases: what the citations say
Abstract risk doesn't change behavior — cases do. These are real Indiana incidents, drawn from IOSHA safety orders and news coverage. Each one maps to a specific NFPA 70E requirement that, followed, would likely have changed the outcome. Every one of them started as an ordinary work order on an ordinary day.
Employee burned in arc flash on energized 480V switchgear — Service Electric of Allen County
An employee working on an energized 480-volt AC main switchgear section suffered arc flash burns serious enough to require hospitalization. IOSHA's safety order reads like a 70E checklist in reverse: the company was cited under 1910.333(a)(1) for failing to de-energize live parts before work, 1910.333(a)(2) for inadequate safe work practices near energized equipment, and 1910.335 for failing to use insulated tools around energized conductors — plus a fourth item for not reporting the hospitalization to IOSHA within 24 hours. Three serious violations at $4,900 each and one other-than-serious brought the total to $15,400.
this is the whole front half of the standard in one incident: Article 110's electrically safe work condition policy, Article 120's verification steps, 130.2's narrow justifications for energized work, 130.5's arc flash risk assessment, and 130.7's PPE selection. And note the employer: an electrical contractor. Being in the trade is not a program. Two electrocution deaths in four months at U.S. Steel Gary Works
In June 2016, a 67-year-old electrician was fatally electrocuted at Gary Works, the flagship mill of the Northwest Indiana steel corridor. In late September of the same year, a 30-year-old maintenance worker on a four-person crew was killed while performing maintenance on a crane at the hot strip mill — a death also attributed to electrocution. Union officials publicly tied the deaths to deep maintenance-staffing cuts earlier that year, saying bare-bones crews were being pushed into hazardous work. IOSHA investigated and fined the company. overhead crane maintenance is one of the classic hidden-energy tasks — runway conductors and collector rails stay energized after the crane itself is "off," which is exactly why Article 120's approach demands identifying all sources, isolating each one, and verifying absence of voltage before anyone touches anything. The staffing angle matters just as much: when maintenance crews shrink, task planning and job safety briefings are the first things skipped. Worker in elevated bucket contacts energized line — Robert Henry Corporation
An employee of Robert Henry Corporation, working as a contractor alongside Indiana Michigan Power near Union and Hester streets in Elkhart, was electrocuted on July 13, 2018 when he contacted an energized line from an elevated bucket — while a disconnected line hung from his bucket to the ground below. IOSHA investigated and fined the company $7,000 for safety violations. line work sits on the jurisdictional seam between 1910.269 and 70E's core scope, but the failure pattern is universal: a conductor treated as controlled when it wasn't, in a work zone where an uncontrolled path to ground was hanging in plain sight. Article 130's overhead line provisions and the pre-job briefing exist for exactly this configuration — and Article 110's host/contract employer duties mean the utility and the contractor each owed the other hazard information. Boom truck operator burned raising overhead lines for an oversized load
A 26-year-old construction worker was critically injured on U.S. 27 in Allen County while operating a boom truck to lift overhead power lines so an oversized load could pass beneath them. He sustained severe burns to both arms and was airlifted to a Fort Wayne hospital in critical condition; two utilities assisted in the rescue. Escorting oversized loads under energized lines is a routine, scheduled task across rural Indiana — and it puts non-electrical workers inside approach boundaries with elevated equipment, the single deadliest combination in U.S. electrical fatality data.
nobody raises an energized conductor with a boom as a "workaround" — that task belongs to the utility, with qualified workers, rated equipment, and minimum approach distances under 1910.269. For everyone else, Article 130's overhead line boundaries treat elevated equipment near lines as a planning problem: locate the lines, call the utility, and keep unqualified persons and machines outside the boundary. If your crews escort loads, trim trees, pour concrete, or roof buildings near lines anywhere in Indiana, they are inside your electrical safety program's scope whether or not anyone calls them electrical workers.

Roofing crew exposed to energized circuit — A&M Home Services
During a residential re-roofing job on Corby Boulevard in South Bend, IOSHA inspectors found employees working around an energized 240-volt circuit without protection from electric shock, alongside missing fall protection at roughly 18 feet and defective ladders. The company was cited under 1926.416(a)(1) and 1926.416(a)(3) for the electrical exposures plus three fall and ladder items — five serious violations, $10,200 total. No one was hurt. That's the point of including it: this is what an IOSHA construction inspection finds before the incident, and it's the same fact pattern that kills roofers and laborers every year.
nothing here required an engineer — it required someone to ask "what on this site is energized, and who might touch it?" before work began. That is Article 110's risk assessment and job safety planning duty applied at its most basic level, and 110.4's training requirement scaled to unqualified workers: awareness of the hazard, the boundaries, and the rule that you don't work around live parts that could have been de-energized. Small residential contractors are exactly the employers most likely to have zero written electrical program — and IOSHA inspects them.

The pattern across all five: not one of these was an exotic failure. Switchgear worked hot. A crane whose runway nobody isolated. A line treated as dead. A boom lifting conductors that should never have been touched. A live circuit over a roofing crew's heads. The 2027 edition's changes — job safety planning, the additional-person requirement, tightened host/contractor duties — are aimed at exactly these ordinary moments.
04Licensing in Indiana: the patchwork problem
Here is the fact that surprises out-of-state employers most: Indiana has no statewide electrician license. There is no state electrical board and no state journeyman or master credential. Licensing happens at the municipal and county level — Indianapolis, Fort Wayne, South Bend, and Evansville each run their own programs, with their own classifications, exams, and renewal rules, and dozens of smaller jurisdictions add their own registration requirements. An electrician can be licensed in Fort Wayne, unlicensed ten miles outside it, and subject to a completely different rulebook in South Bend. Three implications for your electrical safety program:

In a no-state-license state, "qualified person" is entirely your program's word. OSHA's and 70E's qualified person concept requires demonstrated training on the hazards of the specific tasks and equipment: shock hazards, arc flash hazards, boundaries, PPE, and the skills to distinguish live parts and verify de-energization. In Texas you at least start from a state license and build up. In Indiana there may be no license at all — which means your training matrix, your task-by-task qualification records, and your annual retraining documentation carry the entire load. When IOSHA asks who authorized a worker to open that panel, "he's been doing this for twenty years" is not an answer.
05Industry by industry: where 70E does the work in Indiana
Steel (Gary, East Chicago, Burns Harbor — the Calumet corridor)
Northwest Indiana is the largest steelmaking region in the United States — Gary Works, Burns Harbor, and the East Chicago mills between them run some of the heaviest industrial electrical infrastructure in the country: medium-voltage distribution feeding arc furnaces and rolling mills, DC drive systems, and hundreds of overhead cranes whose runway conductors stay hot when the crane doesn't. Automotive and RV manufacturing (Lafayette, Princeton, Greensburg, Fort Wayne, Elkhart)
Indiana builds vehicles at scale: Subaru in Lafayette, Toyota in Princeton, Honda in Greensburg, GM's truck plant in Fort Wayne, plus a deep tier-supplier base — and Elkhart County builds the overwhelming majority of North America's RVs. Electrically these are two different worlds. The auto plants run sophisticated programs but face the classic conditions the 2027 additional-person rule stresses: lean night-shift maintenance crews, robot cells, and pressure to troubleshoot energized rather than stop a line. The RV sector is the opposite profile — hundreds of mid-size plants, high turnover, seasonal surges, and thin EHS staffing, where the honest question is whether a written electrical safety program exists at all. Pharmaceuticals and life sciences (Indianapolis, Lebanon)
Indianapolis anchors one of the country's major pharmaceutical manufacturing bases, with multi-billion-dollar expansion underway in the region. Validated utilities change the electrical safety conversation: clean utilities, HVAC, and process equipment can't simply be switched off without triggering quality events, so the pressure to work energized arrives dressed up as regulatory necessity. It isn't. 130.2's justifications for energized work are narrow, and "shutdown is inconvenient for the batch record" is not among them — the answer is engineering and scheduling: redundant feeds, planned outage windows tied to campaign changeovers, and commissioning-phase decisions that make future de-energization possible. Logistics, warehousing, and data centers (Indianapolis, the I-65/I-70 crossroads, northern Indiana)
Indiana markets itself as the Crossroads of America, and the freight economy proves it: massive distribution campuses ring Indianapolis and line the interstates, dense with conveyor systems, charging infrastructure for lift-truck fleets, and battery rooms — dc hazards (Article 310) and battery work (Article 360) that generic LOTO programs miss. Layered on top is a fast-moving data center buildout, with hyperscale campuses under construction across northern and central Indiana. Rural Indiana's electrical hazards are distinctive and badly underestimated. Grain elevators and co-ops combine aging motor and control infrastructure, dusty classified atmospheres where an arc is an ignition source, and confined working spaces around bucket elevators and dryers. Unlike Texas and other federal-plan states, Indiana's public employers are fully covered by IOSHA — a municipal electric utility, a school district maintenance department, and a county highway garage can all be inspected and cited. That's the stick. The gap I see is that public-sector electrical work spans everything on this page in miniature: water plant MCCs, school kitchen panels, traffic signal work near energized lines, wastewater VFDs — usually maintained by small crews with no written electrical safety program behind them. 06How the 2027 edition lands in Indiana
Every change in the 2027 Implementation Guide applies here, but four hit Indiana employers with particular force:
